U.S. Passport Book/Card
In July of 2001, Bloomingdale Township became a Passport Acceptance Facility. As an Acceptance Facility, the Township accepts applications & fees, and administer the oaths. The Township then priority-mails the completed applications off to a national processing center where they are further reviewed to produce the actual passport book or card.
In July 2008, the passport card was first made available. The card is the wallet-size travel document. It is convenient and costs less, but can only be used to re-enter the United States at land border-crossings and sea ports-of-entry from Canada, Mexico, the Caribbean and Bermuda. It is not valid for international travel by air.
Applications are accepted by appointment only. Hours for appointments are Monday through Friday 8:30AM - 3:30PM. Please call the Supervisor's office at 630-529-7715 to schedule one.
The best starting place for any passport information is the website of the U.S. Department of State Bureau of Consular Affairs at travel.state.gov This site has a very user-friendly interface to allow you to complete the step-by-step DS-11 Application, review it, and print it out. Their website also guides you through which forms may be needed by asking the most relevant questions. Blank PDF copies of forms are also available for print out (to be completed in black ink). Their site provides the most recent processing times, fee explanations, and even allows you to check up on the status of your submitted passport applications.

What you will need to acquire a new passport book/card is as follows:
- Original certified copy of your birth certificate with raised seal or original naturalization papers. No copies can be accepted. These original documents must accompany your application and will be returned to you by the State Department. TIP: This requirement of an original birth certificate with a raised seal is by far the most often ignored requirement when folks come to visit us, leading to the necessity of a second follow-up appointment.
- Valid personal identification such as a driver’s license.
- Social security number.
- A completed and unsigned application. All applications must wait to be signed while here in the presence of an authorized passport agent.
- One 2" x 2" size passport photograph per these specifications: passport photo help TIP: Worth reviewing. Photo specs are our second most common issue.

If you want to renew a passport book/card you may be able to do so yourself by mail without visiting an acceptance facility. Visit travel.state.gov to guide you through a few simple questions to see if you are eligible to file the renewal form DS-82 by mail. In general, if you can answer yes to all six of the following questions, you should be eligible:
- You possess and can submit your most recent passport.
- Your current passport was issued less than 15 years ago.
- You were at least 16 years of age when your most recent passport was issued.
- You are using the same name as your most recent passport or have a marriage license or court order that you can submit as proper documentation to reflect the name change.
- Your most recent passport has not been mutilated, damaged, lost, stolen, or subsequently found.
- Your most recent passport was not issued as Limited Validity.
==>If your answer is no to any of the above 6 questions, you will need to apply in person for a new passport book/card with form DS-11.
==>Renewal fees for adults eligible to apply by mail using form DS-82, “Application for a U.S. Passport by mail,” are $130 for the passport book and $30 for the passport card.
Cost for a new passport book for adults is:
- $130.00 check or money order payable to the U.S. Department of State (TIP: Since payment gets mailed off along with application, it must be check/money order)
- $35.00 cash or money order payable to Bloomingdale Township
Cost for a new passport book for minors is:
- $100.00 check or money order payable to the U.S. Department of State (TIP: Since payment gets mailed off along with application, it must be check/money order)
- $35.00 cash or money order payable to Bloomingdale Township
Cost for a new passport card for adults is:
- $30.00 check or money order payable to the U.S. Department of State (TIP: Since payment gets mailed off along with application, it must be check/money order)
- $35.00 cash or money order payable to Bloomingdale Township
Cost for a new passport card for minors is:
- $15.00 check or money order payable to the U.S. Department of State (TIP: Since payment gets mailed off along with application, it must be check/money order)
- $35.00 cash or money order payable to Bloomingdale Township
Both the passport book and card are valid for 10 years for adults (5 years for minors under age 16). The passport book/card will take multiple weeks to receive after the Department of State receives your application. Slightly quicker processing, called "Expedite", is available for an additional fee of $60. Visit travel.state.gov for the current estimates of processing times. 7 - 10 days after a DS-11 application has been applied for, you may track its status at https://travel.state.gov/content/travel/en/passports/need-passport/status.html. This is especially helpful if your travel plans are nearing and you wish to consider expediting fees.
