| U.S. Passport Book/Card
In July of 2001, Bloomingdale Township became a Passport Acceptance Facility. While the Township does not issue the passport book/card, it can accept applications and administer oaths. The passport card was first made available in July 2008. The passport card is the wallet-size travel document that can only be used to re-enter the United States at land border-crossings and sea ports-of-entry from Canada, Mexico, the Caribbean and Bermuda. 
It is not valid for international travel by air.
Applications are accepted by appointment only. Days for which appointments can be made are Tuesday, Wednesday and Thursday.
Applications are available at the Township offices or may be downloaded by visiting the U.S. Department of State Bureau of Consular Affairs website at www.travel.state.gov
What you will need to acquire a new passport book/card is as follows:
1) Original certified copy of your birth certificate with raised seal or original naturalization papers. No copies can be accepted. These original documents must accompany your application and will be returned to you by the State Department.
2) Valid personal identification such as a driver’s license.
3) Social security number.
4) A completed and unsigned application. All applications must be signed in the presence of an authorized passport agent.
5) Minimum of two passport size photographs. Size should be 2” x 2”.

If you want to renew a passport book/card you may be able to do so yourself provided you answer yes to all of the following questions:
1) You possess and can submit your most recent passport.
2) Your current passport was issued less than 15 years ago.
3) You were at least 16 years of age when your most recent passport was issued.
4) Your are using the same name as your most recent passport or have a marriage license or court order that you can submit as proper documentation to reflect the name change.
5) If your answer is no, to any of the above questions, than you would have to apply for a new passport book/card and follow those requirements.

Renewal fees for adults eligible to apply by mail using form DS-82, “Application for a U.S. Passport by mail,” are $110 for the passport book and $30 for the passport card. The Passport book/card can take four to six weeks before receiving. Both are valid for 10 years for adults and 5 years for minors under age 16.
